Intentional Misuse Notice
Any intentional misuse of this form will be reported to the proper authorities. This form is designed for the sole purpose of those University of Arizona students who are currently participating in band classes. All other use is prohibited and is considered intentional misuse.
Please fill out all information requested completely and submit it to the Band Office prior to the rehearsal, if possible. It will not be accepted if submitted later than one week after the missed rehearsal.
